If you would be interested in purchasing the forms here is what to do:
1. Contact us at 800-388-2267 or 513-271-3923 or jheisel@heiselandassoc.com about payment. 2. We need your email address so we can email you a set of completed forms. 3. In order to create a set of forms just for you we need: - the correct spelling of your name - the correct spelling of your business if you are including a business name - your address - your phone numbers, including after hours numbers and/or pager - what license you have and which code of ethics covers you - what your standard fee is - if you have the ability to take credit cards (one of our forms asks for a credit card guarantee. If you don't take credit cards we can eliminate that part of the Statement of Fees) 4. We can email you a completed set of forms, written with your name/business name and phone numbers within a couple of days.
Heisel and Associates takes no legal responsibility for these forms. While the forms have been reviewed and approved by a law firm we cannot guarantee that you will be legally protected by their use. You can have your own lawyer review the forms and/or create a set of forms just for you. We are confident these forms address those issues which we are required to address, but we cannot guarantee that clients will chose not to sue based on these forms. |
